Informed Consent Statement 

The purpose of this study is to investigate the perspective of US health care workers, specifically medical doctors, nurses, nurse aides, nurse practitioners, occupational therapy practitioners, physical therapy practitioners, physician assistants, social workers, speech therapy practitioners, respiratory therapists, and lab technicians, diagnosed with COVID-19. Responding to the survey should take about 15 minutes of your time. Participation is voluntary and you may discontinue at any time during the survey. Please be assured that we “researchers” will not have access to your name, your responses will remain confidential, and we will not be able to connect your responses to your email.
 
Only the researchers will have access to responses to tabulate results.
Both the survey and the responses are sent over a secure, encrypted connection as it is guaranteed by the SurveyMonkey. The data will be entered into a statistical program for analysis, removed from the hard drive, and stored on a secured flash drive.
