Non-emergency patient transport is a free transport service for people who find it difficult to get to hospital appointments on their own and have certain medical conditions or circumstances.

From 28th April 2025, new eligibility criteria for patient transport have been introduced in West Yorkshire and Craven. We want to know if this change has affected you so that we can feed your experiences into a review of the changes by West Yorkshire Integrated Care Board in September. We will do this anonymously without sharing any personal details or experiences that may identify you.

You only need to fill in this survey if you have been refused non-emergency patient transport since 28th April 2025.
