📋 Parent Carer Needs Assessment (PCNA) Survey

If you care for a disabled child or young person, you are legally entitled to request a Parent Carer Needs Assessment (PCNA) from your local authority. This assessment helps understand how caring affects your life and wellbeing and identifies support or services that may help you in your caring role.

We are running this survey to better understand parent carers’ experiences of requesting a PCNA. Your responses will help highlight what is working well and where improvements may be needed. Participation is voluntary, and all information will be treated confidentially. You can stop at any time.

Please do not include any personal or identifiable information in your responses. All answers should be fully anonymised and should not contain names, addresses, or any details that could identify you or another person.
The short version of this survey takes around 5–7 minutes. If you choose to answer the optional additional questions, the full survey takes around 10–15 minutes.
