This survey is designed to help Meyer track the populations that our grants are serving. The survey questions are framed in a way that will enable us to easily aggregate key metrics across all of our grantees. Most organizations collect the data requested in the survey in a more fine-grained way, but we are asking you to summarize it in broad-brush strokes for purposes of this survey. We recognize that not all grantees have sufficient data to answer all of the survey questions, and we have included an answer option for that situation.

The survey responses will be used in an aggregated form to enable Meyer to conduct a summary analysis across all of our grantees. The responses will not be used to assess individual grantees’ progress. All responses are confidential.

If your organization has more than one Meyer grant, a separate survey should be completed for each individual grant.
