North Dakota Health and Human Services Immunization Strategic Planning Project Community Survey

The purpose of this survey is to gather input and opinions from adults about vaccination uptake and access in North Dakota. North Dakota Department of Health and Human Services (HHS) will use the results of this survey and other information to develop the State Immunization Strategic Plan. Your response to this survey will help state and local health groups focus their efforts on working to ensure all North Dakotans can be healthier and happier by staying current with wellness immunizations. The survey takes approximately 10-15 minutes to complete and is completely anonymous. No personally identifying data will be collected.
