FOR PARENTS, RELATIVE CAREGIVERS, GUARDIANS

The Los Angeles Department of Mental Health (DMH), Los Angeles County Office of Education (LACOE), and the University of California at Los Angeles (UCLA) have partnered to evaluate a strategy to organize school and community resources. We are asking all parents/caregivers to complete this survey to help us improve the quality and impact of this strategy for school staff, parents/caregivers, and students. This survey should take approximately 15 minutes to complete and will help to create a better learning environment for everyone at the school.

Participation in this survey completely is voluntary. There are no penalties for declining to participate in the survey or for declining to respond to items on the survey. Your survey response will not be included in your child's academic record.

There are no right or wrong answers in this survey. Please read each question carefully and answer based on your own experiences and opinions. The answers you give are very important. Answer the questions honestly and as best as you can.

This survey is confidential. You will be asked to create a unique ID below by answering a few questions.  This unique ID will be used to link your survey responses across time to identify trends and better understand how, if at all, the Community Schools Initiative is having an impact.  This unique ID cannot be used to determining your identity.  Your responses will be combined with the responses from other individuals and will be reported in aggregate (group) form only.
