Thank you for assisting with the development of a centralized resource of TJR tools and resources
In December 2019, in partnership with Ontario Health (Quality), RCA released a survey to BundlCed Care Providers, asking them to report on their experience with Bundled Care implementation. The results showed that over 90% of respondents reviewed best practice guidelines as part of their planning process prior to implementing bundled care. Having said that, only 63% of respondents reported use of clinician reported outcome measures and even fewer collected patient reported outcomes (47%) and experience measures (18%). During this time of system change, including the provision of care through a pandemic and a transforming health care system, a new way of organizing and delivering care is in development. New and innovative delivery mechanisms are being implemented, including virtual care. In an effort to support the ease of access and sharing of rehabilitative care best practice resources/tools for the hip and knee replacement population and with a focus on improving processes for Bundled Care organizations, the RCA TJR Best Practices Task Group is developing a web based centralized resource of tools and resources which will be shared on the Rehabilitative Care Alliance website.
We are therefore requesting your assistance. If your organization has developed and/or implemented resources or tools that are evidence-based and align with best practices, such as patient & family education materials, care pathways and assessment tools, we would appreciate if you could please do the following:
Complete the following survey and upload each tool/resource below.
You may submit up to 5 tools/resources in this survey. To submit more than 5 tools/resources, please open a new survey to complete your submission.
