Bursary application form.

The Bursary Application includes 27 questions, and you must respond to all questions via this on-line process in order for your application to be submitted and considered.  Two of the questions require uploading documents (your CV; and a letter of support from your association, local regulator, your university, or your employer).  Other questions may require you to do some preparation (for example: deciding which portion of the Congress you plan to attend; and researching estimated air fare, if you are requesting support for that expense). 

In order to help you prepare for the Application process before you begin, you may view or print a list of the questions in advance, here. This version is only to help you prepare; your actual application must be completed and submitted via the Survey Monkey facility.

You must provide a response to every question.  If a question is not relevant to your situation, please indicate "Not Applicable" or explain briefly why you are not providing a response.  If you are not able to provide a CV or a letter of support (as described above), please upload a document explaining that you are not able to provide that information.

You are not required to complete the application process in one sitting. The survey is editable, that is, you can open the survey and change your answers until you feel comfortable with the information provided and you submit the survey.
