The survey should be completed by someone in the school district or state who is knowledgeable about districtwide K–12 “music and arts” instruction. For questions 9-16, please consult with district-level staff who oversee federal programs and would know best about the dollar amount and use of ESSER (Elementary and Secondary School Emergency Relief funds established during COVID), Titles I, II, and IV-A funds in your schools during the 2024-2025 school year. The survey should take approximately 20 minutes to complete.
