Quality of Care for I/DD Adults

Introduction

This survey was designed to measure the quality of available home and community support services in the state of Florida for adults, over the age of 21, born with a developmental disability. These adults are typically referred to as I/DD adults. Your responses are anonymous, unless you choose to provide your contact information, asked for at the end of the survey.

In Florida, a developmental disability is defined by Florida Statute 393.063 as a disorder or syndrome, such as autism, cerebral palsy, intellectual disability, Down syndrome, Prader-Willi syndrome, Phelan-McDermid syndrome, or spina bifida, that manifests before the age of 18 and causes a substantial, long-term handicap.

Responses will be used to help FL families and policymakers understand the quality of care for I/DD adults over the age of 21, who will require support for the rest of their lives, to thrive. You only need to respond to items with an asterisk (*) to submit this survey. Therefore, feel free to leave questions unanswered if you are unsure how to respond.
