Thank you for taking the time to complete this survey. This is your opportunity to contribute to the biggest single report on what it is like to live as a family of a disabled child in Calderdale.
When the survey closes we will compile a detailed report which will:
• Be presented to senior managers and commissioners in Calderdale
• Provide information which can be fed in nationally to influence policy and approach and approach
• Help to shape the way services are provided and influence local priorities
All feedback will be anonymous and your details will not be passed on.
Your voice really can make a difference.
How to complete the survey
This survey looks at life as a family of a disabled child in Calderdale and asks questions relating to people’s general experiences. The survey is split into three categories:
• Support and Outcomes
• Access to Information
• Parent Carer Wellbeing
The survey asks you to score each question by to what extent you agree with each statement. 5 will always be the highest (i.e. agree most) and 1 will always be the lowest (i.e. agree the least).
It will be really helpful if you can make a comment as to why you gave each rating, in the space provided.
Prize Draw
Each completed survey will be entered into a prize draw to win a £50.00 high street voucher.
