Consultation on Resourced Provision in Calderdale

Thank you for taking the time to complete this questionnaire.

Calderdale Parents and Carers is consulting with families (on behalf of the Specialist Inclusion Service in Calderdale) to look at how disabled children and young people can best be supported in mainstream education.

This includes both:

• 'resourced provision' (specialist units attached to a mainstream school supporting children with Autistic Spectrum Conditions, Visual Impairments, Hearing Impairments and physical disabilities)

• and support exclusively within a mainstream school.

How to complete the survey:

There are three main questions, with plenty of space for you to provide as much information as you wish.

All feedback will be anonymous and your details will not be passed on.

When the survey closes we will compile a detailed report which will help to shape the way these services are provided and influence local priorities.
