Softball Experiences Survey

To Project Participant:

You are invited to take part in a research project conducted by Gretchen Peterson, Ph.D., a faculty member at California State University-Los Angeles. I hope that this research will lead to a better understanding of the experiences of adult softball players.

You are being asked to complete the following survey which should take approximately 20 minutes to complete. As with any research project, you are free to discontinue your participation at any time.

There are only minimal risks expected from your participation in this research study. Reports resulting from this study will not identify you as a participant. Your name is not attached in any way to the answers you provide on the survey and so your responses will remain anonymous to the researcher. There is a minimal risk of computer IP addresses being connected with survey responses by the survey company or by computer hacking into the website. However, the survey company does pledge to keep any data collected confidential. Survey responses will be kept for a minimum of three years following completion of the study before being destroyed.
