Survey of Motorcycle Helmet Usage

You are invited to voluntarily participate in a research project investigating motorcycle riding and the use of helmets.
The purpose of the study is to gather information from motorcycle riders about their experience and interests using helmets. Participation in this short survey will take no longer than 10 minutes to complete. Participation will remain anonymous and no identifying data will be collected.
The procedure is a short survey, consisting mostly of multiple choice questions. The questions will tell us a little about who you are, your experiences riding motorcycles, and give us some ideas about how helmets might be improved to better serve your needs. You have the right to refuse to answer any questions that you do not wish to complete and/or answer.
While this research may not directly benefit you, the results will be used to improve the design of motorcycle helmets and the safety of motorcycle riders. There are no known risks to participating in this survey, and all of your answers will be confidential.
No incentives will be provided for completing the survey. All answers will be destroyed after completion of the research and for at least seven years following the completion of any published reports on what we have learned. All information will be used for research purposes only.
If you agree to participate, you must be at least 19 years of age. You can withdraw at any time without consequence. Please contact me at 460-6371 or the Institutional Review Board at the University of South Alabama at 460-6308 if you have questions about your rights as a research participant.
