When Hugh Chatham Health affiliated with Atrium Health Wake Forest Baptist Health, the Yadkin Valley Wellness Foundation was created to help improve community health and wellness in the greater Yadkin Valley region.

The Foundation is making a plan to use its resources in the best ways for the whole community. To do that, we want to learn from people who live and work here about what affects health and wellness in our community.
