Please take a moment to complete the following survey. It will take approximately ten minutes to complete. This survey is being conducted as part of a Community Health Needs Assessment for Wyoming County with the goal of planning health services for Wyoming County residents.
Your answers are confidential and will be used in a larger analysis to assess the healthcare needs of Wyoming County residents. Only the investigators (The Institute research team) will have access to the raw data, which will be stored in a secure and licensed system. The Institute will prepare a report of aggregated findings for the Children’s Service Center and its partners. No identifying information will be shared in the final assessment report, and you may skip any question you do not wish to answer. Please respond to the best of your ability.
