Welcome and Consent to Participate
You’re invited to take part in a research study about transportation needs in New Hampshire. The New Hampshire State Commission on Aging is partnering with representatives from across the state and the team at Impact Consulting to better understand transportation needs, especially of older adults, people with disabilities, and veterans.
- The survey takes about 10 minutes to complete. You may skip any question or stop at any time. Most people complete it in under 10 minutes. Your answers are anonymous unless you choose to share contact information at the end.
- Caregivers are welcome to help or respond on behalf of someone else. We’ll ask you to indicate that on the next page. If you need assistance (including translation or reading support), please don't hesitate to reach out.
- Participation is voluntary. There are no penalties for choosing not to take part.
