The St John NT Excellence Awards celebrate and recognise paramedics, volunteers and staff who demonstrate exceptional leadership, professionalism and compassion in their roles serving our organisation and the community.

Please complete one nomination form per nominee, providing as much detail as possible, including examples, as to how the nominee meets the criteria.

The judging panel will also consider the nominee’s engagement with St John NT, participation in its programs and initiatives, and their ability to demonstrate the organisation’s values: Respect, Integrity, Collaboration, and Empathy.

Providing examples, explain how your nominee is a role model to their peers and the organisation. Provide details of any significant or outstanding contribution the nominee has made and how it makes them eligible for this award.(Required.)
6.Selection criteria 3: St John NT values
Providing examples, explain how your nominee goes above and beyond the call of duty, clearly demonstrating St John NT’s values: Respect, Integrity, Collaboration and Empathy.(Required.)
