Sik-e-Dakh Child and Family Services Community Survey

Thank you for participating in the Sik-E-Dakh child and family services and infrastructure needs and gaps survey!

The following survey is looking for your input to inform a child and family services department needs assessment. The aim of the survey is to find out more from Sik-E-Dakh families about what we need and hope for from our child and family services to support happy and healthy families.

Your responses will be integrated into the needs assessment which will be used by the child and family services team to direct infrastructure projects, and develop programming and services.

The information gathered from this survey will be entirely anonymous. If you provide your email to enter the prize draw for one of 2 $100 gift certificates, your email address will be kept on a separate list from the survey responses to maintain anonymity.
