Member Survey - Physicians & Nurse Practitioners
The following survey is designed to provide insight for the qathet Division of Family Practice and qathet Primary Care Network as we continue to plan for future initiatives, meet the needs of our members and support recruitment and retention efforts. Additionally, the survey is a tool for self-reflection as it provides an opportunity for you to stop and reflect about what is important to you, what works well and where you are facing challenges in your medical practice.
Responses will be kept confidential and reported in aggregate form. Any reporting will ensure that individuals cannot be identified. Survey data will be used for evaluation and planning purposes and may be shared in an anonymized form with partner organizations to support service improvement and future planning.
