Description & Consent
This survey is part of an evaluation of the Our Place approach. It aims to better understand your experiences with the Our Place community you are involved in, and how the local school, early learning centre, or programs are supporting you and your family.
The evaluation is being conducted by the Our Place Evaluation Team, with support from Our Place Partnership Managers and Community Facilitators at your site.
Your feedback is important and will help improve the support and services offered through Our Place.
Who should complete this survey?
Parents carers and community members connected to and Our Place site, you must be at least 18 years old or older.
Confidentiality
Your individual participation in this survey is confidential. Only members of the evaluation team will have access to the data. Survey responses are anonymous and will be stored securely on a password-protected computer at Our Place. Data will be retained until the completion of the project, or until it is no longer needed. Only the evaluators will have access to the data.
No information about individuals will be given in any reports arising from this survey.
What participation involves
Participation in this survey is voluntary. Your relationship with the evaluators or Our Place staff will not be affected by your participation.
The survey should take approximately 15 to 20 minutes to complete, and you may choose to exit the survey to without submitting your responses. Once you have submitted your answers, you will not be able to withdraw from the evaluation.

How the results will be used
The survey responses will be collated and incorporated into a report for Our Place. The findings from this survey will improve understanding of the effectiveness of Our Place.
