We will follow up with you via email to book your interview using the information you provide in this form. The location will be provided in an email 24 hours prior to your interview.

Interviews are set for one hour, but we acknowledge that individuals may need more or less time. A Cultural Practitioner will be on-site during the interviews as there may be risks involved with being interviews such as triggers.

An honorarium will be provided at the end of the interview. Transportation to and from the interview location is not provided. Taxi chits can be provided to assist with transportation costs. Please fill out the last question of this form if you require this support.
