Help us plan for our health and ageing needs

The District Council of Lower Eyre Peninsula (DCLEP) has partnered with the Cummins District Community Bank® in a project looking at the health and ageing needs of our community.
 
This survey aims to ensure that local health services are maintained and improved by looking at how and where we access services and any gaps or barriers. The survey also examines the needs of our ageing residents, including in-home support and residential aged care.

This survey is open to all residents of DCLEP and we are requesting one survey to be returned per household.
