Providence Patient and Family Partner Gratitude Award

The Providence Patient and Family Partner (PFP) Gratitude Award is an award developed by PFPs. This Award recognizes and thanks an outstanding team or individual at Providence for providing the best possible care experience for their patients, clients, and residents.

Nominations will be reviewed by a panel of PFPs, who will select one team or individual to be recognized at an awards celebration on September 17, 2026.

From May 4 – August 4, 2026, patients, residents, caregivers and family members are invited to complete our online form to nominate a Providence team or individual that has demonstrated outstanding commitment to one or more of the statements outlined in our Care Experience Charter.

What makes a team?
In the context of the award, a team is a group of Unity Health staff or personnel from the same unit or across departments, or an individual who works across multiple units/departments who work collaboratively to provide care, support services, or operations that contribute to the well-being of patients, residents, and families.

Please answer all questions using as much detail as possible (and provide specific examples, where possible).

Last year’s recipient was Marnellie Hilao from the Houses of Providence, who was presented with her award at a ceremony of her peers in September of last year.
