Community Survey: DeWitt County Needs and Resources Assessment for Mental Health, Substance Use, and Intellectual/Developmental Disabilities
The DeWitt County Mental Health Board and many partner organizations invite individuals 18 years and older to participate in the following Community Survey. The purpose of this survey is to become more aware of needs and resources for people who experience mental health challenges, problematic substance use, and intellectual/developmental disabilities. Your participation will help organizations to prioritize, plan, and enhance services and programs for DeWitt County residents.
This survey should take approximately 15 minutes to complete and includes multiple choice and short answer questions. Your participation is completely voluntary; by completing the survey, you are consenting to participate. You can skip questions or discontinue the survey at any point. We do not anticipate any risks to you for taking the survey beyond those that occur in everyday life and the discussion of the above topics.
Your responses will be anonymous; you will not be personally identified. Participant responses will be combined for reporting to project partners and the community.
