Our Future, Guided by Your Feedback

The Stratford General Hospital Foundation is developing a new strategic plan to help guide our work over the next several years, and we want to hear from our community.

Whether you've received care at the Huron Perth Healthcare Alliance - Stratford General Hospital, supported the Foundation, participated in one of our events, or are simply interested in the future of local healthcare, your perspective matters.

This brief survey will help us better understand our community's awareness of the Foundation, what matters most when it comes to local healthcare, and how we can continue to strengthen our impact.

The survey should take approximately 5–7 minutes to complete. Your responses will remain confidential and will be combined with those of other participants to help inform our planning.
