Caregiver Experience Survey

Every four years, the Riverside County Office on Aging develops its Area Plan on Aging to identify the needs of older and disabled adults, as well as any potential barriers to services.  This survey helps us gain valuable feedback from community members about their role as a caregiver or care recipient. 
 
Only one survey is needed per participant.  All survey responses are CONFIDENTIAL, and your participation is VOLUNTARY.  If you come across a question that you do not wish to answer, please select “Decline to state.”  If you have already completed this survey, you do not need to complete it again.
