CAHSAH's Annual Joseph Hafkenschiel Awards Ceremony recognizes individuals who deliver exceptional care and make a meaningful impact in home-based care.
Each year, honorees are nominated by their peers and celebrated at the CAHSAH Annual Conference & Expo - high-lighting the dedication, leadership, and innovation that define care at home.
This year, up to seven honorees will be selected.

Award Categories
Loise C. Lillick Award – Association Leadership
Recognizes individuals who have made significant contributions to the development of home care associations at the local, state, or national level.
Eligibility: Open to members and non-members
CAHSAH Physician of the Year
Honors a physician who demonstrates excellence in home care and actively advances its role within the broader healthcare system.
Eligibility: Open to members and non-members
Lillian H. O’Brien Supervisor Award
Awarded in the following categories: Home Care, Home Health, and Hospice.

Recognizes first-line supervisors who motivate teams and drive excellence in care delivery.
Eligibility: Must be employed by a CAHSAH member organization
CAHSAH Ambassador Award
Honors individuals who actively promote CAHSAH’s mission through advocacy, education, and community engagement.

· Advocacy and outreach
· Leadership and mentorship
· Advancing awareness of home care
Support Service Award
Recognizes individuals or teams providing exceptional non-clinical support that strengthens operations and enhances patient care.
Recognized for:
· Operational excellence
· Process improvement
· Supporting staff and caregivers
Before You Begin
Most nominees must be affiliated with a CAHSAH member organization. The nomination process takes approximately 5–10 minutes. Be prepared to share a brief example of the nominee’s impact.
