Only one survey per person is allowed. Businesses are not permitted to fill out or submit ballots on behalf of their employees, customers or anyone else — with or without consent. Any business offering discounts or anything of value in exchange for votes will be automatically disqualified. The required fields of information must be filled out for the ballot to be eligible. All responses are confidential, and no personal information will be shared with any third parties. Participation conveys consent to contact.
