Introduction
CQI+ is a continuous quality improvement program that aims to empower a positive safety culture in community pharmacies across Alberta. In line with this, the Alberta College of Pharmacy (ACP) is seeking your input based on your own experiences across three separate domains: Teamwork, Safety Culture, and Working Conditions. Your anonymized, aggregated data, collected in this Safety Attitudes Questionnaire (SAQ), will inform ACP about the current attitudes around safety in community pharmacies and will serve as a foundation to support continuous advancements in safety culture.
All pharmacy team members have valuable insights to contribute and are encouraged to participate. Participation in this survey is optional. You are free to withdraw your participation at any time during this survey. This survey is typically completed within five to ten minutes.
This SAQ was developed by the Institute for Safe Medication Practices Canada (ISMP Canada) in collaboration with the Accessing Centre for Expertise (ACE) and an expert advisory panel consisting of representatives from provincial regulatory authorities, academic institutions, and research organizations. It is an evidence-informed tool that explores the safety culture maturity within pharmacy practice. Questions from validated tools have been adopted or adapted to create this SAQ.1,2,3
