Informed Consent

The National Foster Care Youth & Alumni Policy Council is collecting information to learn more about the experiences of young people in and from foster care or out-of-home care. The purpose of this informed consent form is to provide you with information about this effort and invite you to be part of it by completing a poll. Please take time to read the entire form and ask any questions you have before deciding whether or not to take part in this project. All participants must be age 18+ to participate.

You are being asked to participate in this project because of your experience in foster care or out-of-home care. Your participation is entirely voluntary. If you agree to complete the poll, you can stop at any time and for any reason. If you choose not to participate, it will not affect your status or relationship with the National Policy Council.

If you agree to take part in the project, completing the poll will take approximately 10-15 minutes. Information gathered will be used to inform the National Foster Care Youth & Alumni Policy Council on what is working and what isn't working for young people in and from foster care so that they can educate policymakers and stakeholders. The poll may include some questions that seem personal or upsetting. Remember, you do not have to answer any question you do not want to answer and can end your participation at any time. The report will describe the information you and other poll respondents provide. Your responses will be kept anonymous, and no information that could identify you will be included. The information you provide will be used only for this project, but we may have to share this information if it is required by law. All information will be stored securely and will be destroyed two years after project completion.

Findings from the poll will be summarized in a priority on the topic of Prevention to be released in 2026. Beyond helping to improve services for children and families, you will receive no direct benefit for your participation in this project.

Overview: This poll asks about what was happening before you entered foster care or another out-of-home placement, what support could have helped, and whether adults asked for and used your ideas. It also asks about leaving care if that applies to you.

Some questions mention unsafe situations, family separation, and unmet needs. You may stop at any time. There are questions required to submit the poll, but every one includes an answer such as “I don't know or don't remember” or “Prefer not to answer.”
