We are currently gathering evidence and feedback from parents/guardians about how you book appointments for your child at their GP surgery or access their digital medical records, to determine whether you are facing any form of disadvantage because you are part of a serving family. For the purpose of this questionnaire, this access could be through the NHS App, MyCare.Scot or your GP's online systems.
This short questionnaire is designed to be completed by either the serving or non-serving parent or guardian of any children under the age of 18 and should take no more than 5 minutes to complete.
