Please note that whilst we will do our best to meet all your requirements, all programmes are subject to staff and instrument availability.

8.Please give as much additional information about the programme that you would like, including the instrument/voice or whether it is for curriculum teaching/rap and lyricism/music technology etc. and the age of students you would like it for.

10.Please add any additional information here, including how many hours you would like to request for each programme. Please note that a minimum programme length of 2 hours is recommended. We cannot guarantee staffing for programmes that are less than 2 hours in duration.

12.If you would like to cancel any LM provision that you have had during 2025-26, please enter details here. Please note, that all programmes will automatically roll into the next academic year unless they are cancelled by 22 May 2026.

13.Reasons for cancelling programmes (optional - but it is useful for us to have feedback). Please note that if you have approached a Lewisham Music tutor to work directly for your school that you should inform us of this as there is a Tutor Contract Restriction clause in the terms and conditions in the SLA that schools hold with us.
