CCHD Community Health Needs Assessment Survey

This survey is being conducted by the Central Connecticut Health District, the local health department serving Berlin, Newington, Rocky Hill, and Wethersfield. We want to hear your thoughts about your health and the health of your community. Your input helps us understand community strengths, challenges, and priorities so we can better support residents.

This survey has two parts:
Part One: Community Health Concerns
Part Two: Demographic Information

Part One is divided into two sections. The same list of topics appears in both sections:
1. Section One: Rate how important each issue is to you personally.
2. Section Two: Rate how difficult it is to obtain or access services or resources related to the topics in Berlin, Newington Rocky Hill, or Wethersfield.

This survey should take less than 10 minutes to complete.

Participation is voluntary and confidential. Please complete this survey only once. If you prefer not to take the survey online, a paper copy of the survey is available at our main office at 2080 Silas Deane Hwy, Suite 100, Rocky Hill, CT 06067.
