Health Innovation Group's National Practice Facilitation (PF) program is designed to build capacity and train practice facilitators in using quality improvement methodologies to introduce, test, and implement high-impact changes in primary healthcare- supporting practices in building organizational capacity for continuous improvement and advancing towards Patient's Health Home. The program includes learning sessions and mentorship with experienced faculty, access to HIG’s expansive tool and resource library, and connection to the peer-to-peer practice facilitation network.
