Liability Waiver, Release of Claims & Assumption of Risk Diamond & Dogtags Softball Tournament — Front Range Families Classic September 12, 2026 | Keli McGregor Field at Blue Sky All-Star Park, 200 S Harlan St, Lakewood, CO 80226

- Voluntary Participation & Assumption of Risk: I understand that my participation in the Diamond & Dogtags Softball Tournament and associated activities (the "Event") is entirely voluntary. I knowingly assume all risks associated with participation, whether known or unknown, including risks arising from physical exertion, contact with other participants or equipment, weather conditions, slips, trips, falls, and other hazards inherent in outdoor athletic activities.

- Release of Liability: In consideration of being permitted to participate, I, on behalf of myself, my heirs, and assigns, hereby release, waive, and discharge the following parties (collectively, the "Released Parties"): Quality Technology Services, LLC (QTS); Gold Crown Foundation, Inc.; Aurora Chamber of Commerce; Buckley Spouses Alliance; Colorado Christian University; and all event volunteers, staff, sponsors, and representatives acting on behalf of the above — from any and all liability, claims, or causes of action arising from any loss, damage, injury, or death sustained while participating in or attending the Event.

- Indemnification & Medical Authorization: I agree to indemnify and hold harmless the Released Parties from any loss, liability, or expense arising from my participation. I authorize the Released Parties to obtain emergency medical treatment on my behalf if I am incapacitated, and I represent that I am in adequate physical condition to participate.

- Photo & Media Release: I grant permission to the Released Parties to photograph or record my likeness and that of any minor in my care during the Event, and to use such images for promotional and community outreach purposes without compensation.

- Governing Law & Acknowledgment: This waiver is governed by the laws of the State of Colorado. I have read this document in its entirety, understand that it is a binding legal agreement, and voluntarily give up substantial legal rights, including the right to sue the Released Parties. If registering on behalf of a minor, I confirm I am their parent or legal guardian and have authority to execute this waiver on their behalf.
