As we develop our Strategic Plan 2027-2031, we want to listen carefully to the voices of people we support, their circle of supports, staff, and those who know and care about them.
We recognise that, along with families and natural supports, staff play a unique and invaluable role in the lives of people we support. Many have spent years, and often decades, supporting, advocating for and standing alongside people we support through life’s opportunities, challenges and transitions. Others work in office or corporate roles to ensure the smooth running of the organisation.
This survey is about helping us understand what matters most in the lives of people we support and their families, what a good life looks like, and how Horizons can work in partnership with people we support, families, staff, communities and other stakeholders to make this possible. We also want to hear from you as staff about what you like about working in Horizons and what we could improve upon as your employer.
We would really appreciate it if you could take the time to complete this questionnaire, and to only complete one questionnaire. All completed questionnaires, along with a series of focus groups, will be used to inform our next strategic plan. Together we can ensure that the next strategic plan is in line with national and international best practice, the UN convention on the Rights of Persons with Disabilities (UNCRPD), and National Policy and Regulations.
This survey is anonymous. We do not collect names, email addresses, or IP addresses. Responses are used only to inform our Strategic Plan 2027-2031. Participation is voluntary, and you may stop at any time.
