The Appalachian Center of Hope (ACH), supported by the Omni Institute, is engaging in a comprehensive strategic planning process to better understand the needs of our community and define our priorities for the next three years. The purpose of this survey is to better understand community perceptions of substance use, stigma, and recovery — and to identify the gaps and barriers people face when seeking or supporting access to substance use and recovery services in our area. As a community member of Southwest VA, your input and perceptions are extremely valuable to this process.

This survey should take you no more than 15 minutes to complete. It is voluntary and confidential — we do not ask for any information that could be used to identify you, and all responses will be grouped together in a report.
