The Club / Diner Member Application |
Please read before beginning this application.
This Membership Application is for The Club and/or Diner. You can apply for one or both programs. Age limits apply.
This application is secure and will take approximately 5 minutes to complete.
The Club membership fee is $20 ($10 for Santa Monica residents). There is no member fee for the Diner (for eligible Santa Monica residents only). Scholarships for both programs are available - please call to inquire.
Required Documents:
A valid picture ID is required for Wise & Healthy Aging membership programs and proof of residency is required. For Santa Monica residents, acceptable proof of residency are utility bills (gas, electric, water, trash) or copy of lease agreement. Telephone or cable bills not accepted. All statements must a have current address. Please do not submit bank or financial information.
Paper application: Applications can also be submitted in person or mailed to Wise & Healthy Aging at 1527 4th Street, Santa Monica, California 90401 - Attention: Member Services. You may also email it to: memberservices@wiseandhealthyaging.org.
