Community Views on Organ Donation and Transplantation

1.Word in Black, a national newsroom and research group dedicated to informing and empowering the Black community, invites you to participate in a brief online survey designed to better understand community views on organ donation and transplantation. If you agree to take part, you will complete an approximately 8- to 10-minute questionnaire consisting of multiple-choice and short-answer items. Participation is entirely voluntary, and you may skip any question that is not marked as required or stop the survey at any time without penalty.

A small set of demographic questions—specifically age, location, and race or ethnicity—is required so we can ensure our findings accurately reflect the diversity within the United States. Each required item includes a “Prefer not to answer” option; however, if you decline to answer any of these questions, the survey will end. Other than possible strong emotions when reflecting on personal topics, there are no foreseeable risks. You will not receive any direct benefit beyond a raffle entry for a prize, which is provided only if you meet eligibility criteria and complete all survey components as instructed.

Your responses will be stored securely on password-protected servers accessible only to authorized members of the research team and will be kept confidential. Results will be reported only in summary form and de-identified, meaning that all identifiers have been removed. The de-identified data will be stored separately from your contact information to protect your privacy, and any email address you provide for payment will be stored separately from your survey data and used solely to process the incentive. If you have questions about the study, please email research@wordinblack.com.

You must be 18 years or older to participate. By selecting “I consent to participate,” you confirm that you are at least 18, have read the consent form, and voluntarily agree to take part in the study. If you do not wish to participate, please select “I do not consent to participate.”
