Welcome and Thank You
At Horizons, we believe that every person should have the opportunity to live a good life of their choosing, enjoy meaningful relationships, experience of belonging in their community, exercise their rights, contribute as a citizen, and be recognised for who they are.
As we develop our Strategic Plan 2027-2031, we want to listen carefully to the voices of people we support, their parents, guardians and other family members, and those who know and care about them best.
We recognise that families and carers play a unique and invaluable role in the lives of their loved ones. Many families have spent years supporting, advocating for and standing alongside their child through life's opportunities, challenges and transitions. We acknowledge the expertise that comes from this lived experience and the commitment, knowledge and care that families and natural supports bring every day.
We also recognise that many families and carers continue to navigate complex systems and face ongoing concerns about the future. We are grateful for the opportunity to learn from your experiences, your insights, hopes and dreams.
This survey is about helping us understand what matters most in the lives of children and families, what a good life looks like, and how Horizons can work in partnership with people we support, families, carers, guardians, communities and other stakeholders to make this possible.

We would really appreciate it if you could take the time to complete this questionnaire, and to only complete one questionnaire. All completed questionnaires, along with a series of focus groups, will be used to inform our next strategic plan. Together we can ensure that the next strategic plan is in line with national and international best practice, the UN convention on the Rights of Persons with Disabilities (UNCRPD), and National Policy and Regulations.
This survey is anonymous. We do not collect names, email addresses, or IP addresses. Responses are used only to inform our Strategic Plan 2027-2031. Participation is voluntary, and you may stop at any time.
