Questionnaire for Family Members and Natural Supports
Horizons Strategic Plan 2027–2031

Welcome and Thank You

At Horizons
, we believe that every person should have the opportunity to live a good life of their choosing, enjoy meaningful relationships, experience of belonging in their community, exercise their rights, contribute as a citizen, and be recognised for who they are.

As we develop our Strategic Plan 2027-2031, we want to listen carefully to the voices of people we support, their families, and those who know and care about them best.

We recognise that families and natural supports play a unique and invaluable role in the lives of their loved ones. Many families have spent years, and often decades, supporting, advocating for and standing alongside their family member through life's opportunities, challenges and transitions. We acknowledge the expertise that comes from this lived experience and the commitment, knowledge and care that families bring every day. Natural supports are the informal, unpaid relationships and supports available to a person through their family, friends, peers and community. Natural supports can support the person with inclusion, independence, choice and participation in everyday life, alongside the support they receive from family and from formal services.

We also recognise that many families continue to navigate complex systems and face ongoing concerns about the future. We are grateful for the opportunity to learn from your experiences, your insights, hopes and dreams.

This survey is about helping us understand what matters most in the lives of people and families, what a good life looks like, and how Horizons can work in partnership with people we support, families, communities and other stakeholders to make this possible.

We would really appreciate it if you could take the time to complete this questionnaire, and to only complete one questionnaire. All completed questionnaires, along with a series of focus groups, will be used to inform our next strategic plan. Together we can ensure that the next strategic plan is in line with national and international best practice, the UN convention on the Rights of Persons with Disabilities (UNCRPD), and National Policy and Regulations.

This survey is anonymous. We do not collect names, email addresses, or IP addresses. Responses are used only to inform our Strategic Plan 2027-2031. Participation is voluntary, and you may stop at any time.
