Northwestern Alberta’s Vital Signs Survey

3% of survey complete.
If you live, work or go to school in the City of Grande Prairie, the County of Grande Prairie or the MD of Greenview, or the surrounding communities, we invite you to have your say about the quality of life in your community.

The voluntary survey is open to anyone age 15 and over. (If you are in Grade 9 and are not yet 15, we invite you to participate.)

The Community Foundation of Northwestern Alberta's Vital Signs is a snapshot of the quality of life in local communities. Understanding your community's greatest strengths and challenges will help guide local decision-making  and will give you a chance to shape your community. Your views and priorities are vital to this "community check-up."

- Your responses will remain confidential.
- If there are questions you don't wish to complete, please feel free to skip them and move along to the next.

As our way of saying thanks for filling out the survey, at the end of the survey you may enter your name to win a gift basket valued at $50!

If you have questions, please call the Community Foundation of Northwestern Alberta at 780-538-2820 or email

Thanks for participating! We welcome you to forward the survey link to your friends and colleagues. Results will be released in our 2019 Vital Signs report this October.
