What are the most important health and well-being issues in the communities you serve? WellFlorida Council, the local health planning council, in partnership with numerous community organizations and groups invites you to answer this Community Health Assessment survey. As a healthcare provider, social service (or related service) provider, and/or community partner, we need your input on the most pressing health-related issues facing Columbia County and Hamilton County now and beyond 2026. The survey will be available from February 14 through March 31, 2026. Community leaders will use your answers to take action towards a healthier community.
This survey has eight core questions with some additional items depending on your answers and some demographic questions. It should take no more than 10 minutes to finish the survey. Your answers cannot be used to identify you. Please answer the survey only once.
To be eligible to take this survey:
· You must be at least 18 years old and
· Provide healthcare, social and related services to residents and/or be a partner to community health and well-being improvement efforts in Columbia and/or Hamilton Counties.
