How to complete this test
If you decide to take this test, you will be asked to fill out a few questions that will ask you some information about yourself and your nicotine use.
It is up to you if you wish to share your results with a health care provider.

- When you complete the test, you need to add up your question scores to find out your dependence on nicotine.
- We will not contact you about this test. Your participation only involves completion of the test.
- This test is anonymous**. Do not put your name or any information that may identify you on this test.
Use of this information
Your anonymous* answers and test score may be used with other answers in a presentation or publication.
By completing this test, your consent for the use of your answers is implied.
