To apply for a UHT Hospitalist fellowship Program, please complete this application form and upload the following documents by the application deadline:
  • Curriculum Vitae (CV): A comprehensive CV outlining your education and qualifications, professional and clinical experience, research activities and publications, and teaching experience.
  • Medical Degree: A copy of your medical degree certificate.
  • Specialty Certification(s): Copies of any relevant specialty certificates.
  • In-training evaluations: For trainees who are in a current training program or completed a training program within the last year: All in-training/rotation evaluation forms from your training program.
  • Personal Statement / Letter of Intent: A statement that should answer the following questions: (1) What is your clinical training and work experience and how does it align with hospitalist medicine, (2) Why are you interested in the fellowship at Unity Health Toronto, (3) What are your career goals after the fellowship and how will the fellowship support those goals. This letter should not exceed 500 words.
  • Proof of English proficiency: If English is not your first language (i.e. is not the first language you learned at home as a child), you must provide evidence of English proficiency for admission consideration: a) a valid English test score, or b) an official letter from your medical school or program director, or confirmation on the institution’s website, stating that your degree/training was conducted in English.   • Three Professional Referees: References should be physicians, or physician leaders that you have worked with. If you are currently in training (or completed training within the past year), please include the program director as one of the references. Please note: Referees will be contacted via hmfellowship@unityhealth.to to complete a Referee Evaluation Form.
Note: Incomplete applications will not be reviewed.

Fellowship Intake and Recruitment
The Hospitalist Fellowship Program has two intake periods each year, with anticipated start dates in July and January. Applications are accepted on a rolling basis throughout the year; however, the primary recruitment cycle occurs in July, with an application deadline of September 4, 2026. Recruitment will continue until all available positions are filled.

Selection Process
  1. Review of complete application packages.
  2. Shortlisting of candidates for formal interviews.
  3. Conduct of official interviews.
  4. Final ranking and selection of successful candidates.
  5. Issuance of official offer letters, typically by October, depending on the recruitment process.
