Welcome to the UltraFlora® Complete Women’s Probiotic Consumer Perception Study!

Thank you for your interest. Here’s what to expect:

1. Receive the product free of charge. Metagenics will provide the study product at no cost to you.

2. Take 1 capsule daily for 4 weeks. Consistency is key!

3. Share your feedback at key time points (Days 0-15 and 30)

4. Compensation at study completion

Once you finish all surveys, you’ll receive compensation for your participation. Please note that full participation is required in order to earn compensation.

The study will adhere to applicable data protection laws and internal Metagenics policies and procedures regarding data collection, storage and use. As Participant, you will be asked to provide your consent to participate in the Study prior to completing the survey. You may withdraw at any time, and upon withdrawal, your data will be permanently deleted from the dataset and will not be included in the Study.

By choosing to participate in this Study, you acknowledge and agree to the following:

1. Purpose of the 'Study' and Sponsor Disclosure. This 'Study' is designed to gather consumer opinions, perceptions and experiences related to the UltraFlora® Complete Women’s Probiotic ("Product") and may be used to help substantiate marketing claims for the Product. Your responses may be aggregated, analyzed and referenced in support of such claims. This study is not designed to diagnose, treat, prevent, or cure any medical condition.
2. Voluntary Participation. Your participation is entirely voluntary. You may withdraw your participation in the study at any time Any incentive or compensation offered for participation is not conditioned on providing positive feedback.
3. Accuracy of Responses. You agree to provide honest and accurate feedback based on your personal experience with the Product. Your responses should reflect your genuine opinions and perceptions.
4. No Medical Advice. Information provided during this 'Study', including any discussion of Product use, is for research purposes only and should not be considered medical or health advice. You should consult a qualified healthcare professional before making any decisions regarding your health or supplement usage.
5. No Guarantees. Your individual experience may differ from the perceptions or outcomes reported by other participants. Metagenics makes no guarantees regarding the effects, benefits or results of the supplement based on your participation.
6. Product Safety and Personal Responsibility
If you choose to use any product mentioned in this 'Study', participants are responsible for carefully reviewing product labels, warnings, and contraindications. Please follow all instructions provided. At Metagenics, we guarantee that our products are safe when used as directed. If you have any questions or encounter any issues, please inform us promptly.

8. Use of Responses. By participating, you acknowledge and agree:
a. that your survey responses, comments, and feedback may be collected, analyzed, and used by Metagenics for marketing, promotional and informational purposes;
b. your responses may be quoted or referenced publicly, including in advertising, product materials or other promotional content by Metagenics; and
c. any such use will not include your full name or personally identifiable information, unless you provide express written consent.
