The Communities for Children team at Amity Health would like to thank you for taking the time to participate in this survey (approximately 10 minutes). By sharing your experiences, thoughts and ideas as a parent, carer or family member living in the Great Southern, you are helping shape the future of programs and services for local children and families.
Amity Health has been delivering the Communities for Children program, funded through the Commonwealth Government, since 2006. Your feedback plays a vital role in guiding both current initiatives and future planning to ensure our region continues to provide meaningful, responsive and effective support for families.
All responses are confidential. Your individual answers will be de‑identified, and any personal details you provide will be securely stored and accessible only to authorised research staff.
