2026 Epilepsy Foundation of Minnesota Needs Based Survey

EFMN offers support to people and families at all points in their epilepsy journey. The purpose of this survey is to help us better understand the needs of people impacted by epilepsy. The information you provide will be used to design, guide and improve our programs and resources to support people impacted by epilepsy. It is okay for more than one person in your household to complete this survey because you may have different views on the epilepsy journey. Your answers are anonymous and will be combined with others to report on overall findings. This survey should only take about 15 minutes of your time.
