Thunder Bay Regional Health Sciences Centre (TBRHSC) is currently conducting a survey to better understand the healthcare needs of Black people within our community, in order to develop a Black Health Action Plan that is in alignment with Ontario Health. We would like to invite you to participate.

This survey is voluntary and will be kept strictly confidential. Please do not identify yourself in your responses, or share any Personal Health Information (PHI). This information will be used internally at TBRHSC for the purposes of quality improvement.
